PURA syndrome is a rare neurodevelopmental condition caused by variants in the PURA gene. It can shape motor development, muscle tone, communication and feeding — and every child’s profile is unique.
At Rive du Lac, more than ten children with PURA syndrome are part of our cohort — some attend weekly sessions, others come for targeted monthly intensive cycles. Over time, we’ve documented a trajectory of consistent, measurable progress.

In the landscape of rare neurogenetics, a diagnosis is often mistaken for a destination. At Apexa QLA Suisse, we recognise it as the initial site survey. Our clinical philosophy is built on the premise that a genetic diagnosis defines the starting point — not the ceiling. We move away from passive patient management, viewing the child’s neuro-biological profile as a dynamic system primed for structural optimisation.
Leveraging longitudinal insights from a globally significant cohort of more than ten children with PURA syndrome — who attend our centre for interventions ranging from consistent weekly sessions to targeted monthly intensive cycles — we have documented a trajectory of consistent progress.
In this 30-minute clinical overview, Co-Founder Fabienne Theler deconstructs the O.R.C.A. (Objective Reasoning & Clinical Architecture) method: a proprietary framework anchored in advanced clinical reasoning. It strategically integrates a multi-modal toolkit — DMI, TASES, NISE-Stim, Galileo vibration and a diversified range of specialised neuro-interventions — to optimise neuro-motor pathways and facilitate measurable functional gains.

Fabienne Theler, MSc PT (King's College London), is an internationally recognised physiotherapist and clinical thought leader, co-founder of Apexa QLA Suisse and creator of the proprietary O.R.C.A. (Objective Reasoning & Clinical Architecture) method. Together with her team she advances therapeutic care through a distinctive combination of clinical expertise, evidence-led innovation and precise therapy design.
A clinical architect known for setting rigorous, evidence-informed benchmarks in quality and outcomes, Fabienne trained directly with Beverly Cusick — a pioneer of pediatric physiotherapy and global thought leader — over many extended visits to Colorado. She holds Dynamic Movement Intervention (DMI) Intermediate A certification — DMI co-founded by Jo Anne Weltmann and Jake Kreindler, and the highest published DMI level practised in Switzerland. Her work integrates evidence-led methods with outcome-driven clinical reasoning, bringing precision, reproducible structure and measurable impact to neuromotor rehabilitation.
Originally from the Valais region of Switzerland, she brings both precision and perspective to her work. From her centre on the shores of Lake Zug, she and her team design therapy architectures that integrate movement, sensory systems and everyday function into a cohesive whole.
Apexa QLA Suisse is defined by its collaborative culture. Her team — the Therapy Avengers — engages with specialists worldwide through the O.R.C.A. method, encouraging exchange, refinement and shared progress in therapeutic practice.
